Digital odontogram: a practical guide

The odontogram is the visual heart of a dental record: a map of the mouth where you mark what you find and what you plan to do. On paper it is a photocopied diagram; digitally it becomes a living chart tied to the rest of the patient's record. This guide explains what a digital odontogram is and how to get the most from one.

It is written for dentists and clinic staff who are evaluating or starting with digital charting, and it stays vendor-neutral until the last section.

What is a digital odontogram?

An odontogram is a diagram of all the teeth on which you record their condition: caries, restorations, missing teeth, prostheses, and planned treatment. A digital odontogram does the same job on screen, but every mark is data. You can click a tooth to record a finding, see current versus planned states, and have the chart update as treatment progresses, all linked to the patient's history rather than filed in a drawer.

Tooth notation: FDI, Palmer, and Universal

Teeth are numbered with a notation system so that a chart reads the same to everyone. Three are common, and it helps to know which your software uses:

  • FDI (ISO-3950): two digits per tooth, quadrant then position. The most widely used system internationally, including across Latin America.
  • Universal: numbers 1 to 32 for permanent teeth, common in the United States.
  • Palmer: a quadrant symbol plus a number, still seen in orthodontics.

Permanent and primary dentition

A general chart assumes 32 permanent teeth, but pediatric dentistry needs the 20 primary teeth, and children in mixed dentition have both at once. A digital odontogram should switch to a primary or mixed layout so a child's chart is accurate. In FDI notation the primary teeth use quadrants 5 to 8 (for example 51 to 85), and a good chart can default to the right dentition from the patient's age.

What you record on each tooth

Charting is more than 'present or absent'. On each tooth you typically capture:

  • Condition by surface: caries, restorations, or fractures on specific tooth surfaces.
  • Whole-tooth states: missing, extracted, implant, crown, or bridge.
  • Planned versus done: what you intend to treat and what has been completed.

From paper to a digital chart

The value of going digital is not the drawing, it is the link. A digital odontogram sits inside the patient's record next to their history, prescriptions, and images, so a finding you charted last year is one click away this year. It is legible, it does not get lost, and it feeds treatment plans and estimates instead of being copied by hand. The tradeoff is that the data now lives in software, which is exactly why where that software stores it matters.
