Medical Records

Learn how to create, manage, and organize medical records for your patients in HM Praxis.

Local-First Data Storage

Patient medical records are stored on your device. HM Praxis does not transmit patient data to external servers - you retain full control over where records live and how they're backed up.

Creating a Medical Record

Select Patient

Navigate to the patient you want to create a record for. You can do this from:

  • The Patients list (search by name or ID)
  • From an appointment in your calendar
  • From the patient's profile page

Create New Record

Click the "New Record" button from the patient's profile or appointment view. You'll be presented with a form to create the medical record.

Choose the type of medical record you want to create:

Fill in Record Details

Complete the following fields:

  • Title: Brief description (e.g., "Routine checkup", "Root canal - tooth #14")
  • Content: Detailed medical information, findings, or notes
  • Attachments (Optional): Add supporting documents, images, or PDFs

Save the Record

Click "Save" to store the record. The record is immediately saved to your local database and appears in the patient's medical history.

Data Privacy

All medical records are stored locally on your device. HM Praxis does not upload patient data to our servers - only authentication and license status traffic reach our infrastructure.

Structured Records and Templates

Beyond free-text notes, HM Praxis can capture records as structured, typed fields using a record template. A template is a named set of fields (text, number, date, scale, yes/no, or a choice list) that you fill in like a form, so your documentation stays consistent from one visit to the next.

A default for every specialty

HM Praxis ships a tailored default template for each of its 23 specialties, plus universal templates such as a SOAP note (Subjective, Objective, Assessment, Plan). The templates you see are the universal ones plus the one for your own specialty.

Make them your own

Open Settings > Templates to add your own templates or adjust the fields on existing ones. Custom templates use your own field labels and are available the next time you create a record.

Pick the template that matches the visit

When you create a record you can choose a structured template or write a free-text note. Structured fields make later search and reporting far more reliable, so prefer a template whenever one fits.

Records Follow Each Clinician's Specialty

HM Praxis supports 23 specialties, and each member of your team chooses their own. The app then tailors the available record templates to that specialty:

  • A dentist gets dental tools (the odontogram, periodontal charting) and dental record templates
  • A pediatrician gets growth tracking
  • A physiotherapist gets progress tracking
  • Everyone keeps the universal templates such as SOAP and consultation notes
Set your specialty first

You choose your specialty when you first set up the app, and you can change it any time in Settings. The organization never overrides a clinician's personal choice, so a multi-specialty clinic works from one shared database while each provider keeps the right tools. Viewing Medical Records

To view a patient's medical history, navigate to their profile page. All records are displayed chronologically with the most recent at the top.

Search and Filter

You can search records by:

  • Record type (consultation, treatment, prescription, etc.)
  • Date range
  • Keywords in title or content

Edit or Delete Records

Click on any record to view full details. From the detail view, you can:

  • Edit the record content
  • Add or remove attachments
  • Delete the record (requires confirmation)

Attachments

You can attach files to any medical record, such as lab reports, imaging scans, or consent forms. Supported file types include PDF, JPG, PNG, and common document formats. All attachments are stored locally on your device.

🔄Organization Sync (Pro Only)

If you have a Pro subscription with an organization, medical records sync directly between your team's devices over your local network. There is no cloud: patient data never leaves your devices.

How Sync Works

  • One team member shares the organization database with a new device over the local network (a one-time pairing)
  • After pairing, changes sync directly between paired devices on the same network
  • Sync is peer-to-peer over your LAN. No patient data passes through or is stored on any external server
  • Paired team members on the same network see the same medical records
Initial Setup Required

Team members must first copy the organization database via local network. After that, real-time sync happens automatically. See the Data Sync Guide for setup instructions.

Best Practices

  • Use descriptive titles that clearly identify the visit or procedure
  • Choose the appropriate record type for better organization
  • Attach relevant documents (lab results, imaging, consent forms) for easy reference
  • Link records to appointments when applicable for better tracking
  • Use SOAP notes format for structured clinical documentation
