Prescriptions

Keep a structured record of every medication you prescribe. Prescriptions in HM Praxis are tied to a patient, a prescribing staff member, and a list of medication line items with dosage, frequency, and refill rules.

Local records, not transmission

Prescriptions in HM Praxis are recorded for your own clinical file. The app does not transmit to pharmacies or third-party prescribing networks.

Creating a Prescription

Open the Prescriptions Page

From the sidebar, click Prescriptions. The list shows every prescription you have issued, with status pills (active, completed, cancelled, expired). You can filter by patient with the dropdown at the top.

Click "New Prescription"

The new-prescription form opens. Pick the patient, the prescribing staff member, and an optional valid until date.

Add Medication Line Items

For each medication on this prescription, click Add Item and fill in:

  • Medication from your medications catalog
  • Dosage (for example, "500 mg")
  • Frequency (for example, "twice daily")
  • Duration (for example, "7 days")
  • Quantity dispensed
  • Instructions for the patient

One prescription can contain multiple medications, which is useful for combination regimens.

Set the Refill Count

Enter how many refills the patient is allowed (default 0). HM Praxis tracks refills used against this limit, and the prescription auto-completes once the limit is reached.

Save the Prescription

Click Save. The prescription is written locally and appears on the patient's profile under the Prescriptions tab.

HM Praxis is not an e-prescribing system

Prescriptions are recorded for your own files. HM Praxis does not transmit prescriptions to pharmacies, validate against drug interaction databases, or fulfil controlled-substance reporting requirements. Always follow your jurisdiction's rules for the actual prescription hand-off.

Recording a Refill

When a patient asks for a refill on an existing prescription:

  1. Open the prescription from the patient's profile or the main list
  2. Click Record Refill
  3. The refill counter increments and a note is appended
  4. Once refills used equals refills allowed, the prescription auto-completes
Always re-check before refilling

HM Praxis does not enforce refill expiration dates beyond the valid-until field you set. Use the valid-until date when you want to force a fresh consultation before more refills.

Prescription Statuses

Active

The default. The patient can still request refills (if allowed) and the prescription is in force.

Completed

All allowed refills have been used, or you manually marked it complete.

Cancelled

Discontinued early. Use this when a patient stops a medication or you switch them to a different regimen.

Expired

The valid-until date has passed. The prescription stays on the patient record for history but no further refills are allowed.

The Medications Catalog

Before issuing a prescription, the medication must exist in your medications catalog. Each medication has:

  • Brand name and generic name
  • Category (antibiotic, analgesic, etc.)
  • Strength (mg, ml, IU)
  • Form (tablet, capsule, syrup, injection)
  • Manufacturer
  • Whether it requires a prescription

You can build the catalog up over time - HM Praxis ships empty so you only carry the medications you actually use.
